This said, you may STILL be able to utilize your insurance benefits!

How to Check your Out-of-Network Benefits:

    • Locate the customer service number on your insurance card.

    • Follow the prompts to speak with a representative (not an automated system).

    1. Do I have out-of-network benefits for outpatient physical therapy? (Yes or No)

    2. Do I have an out-of-network deductible for outpatient physical therapy?

      1. If yes, how much is it? ( $_____ )

      2. How much has been met? ( $_____ )

    3. Do I have a copay or coinsurance?

    4. Do I need an order or written prescription or written referral from a doctor or other source? (Yes or No)

      1. If yes, will any order/ prescription/ referral be accepted, or does it have to be from a primary care physician?

    5. Does my policy require pre-authorization? (Yes or No)

      1. If yes, do I already have one on file? (Yes or No)

    6. How do I submit my Superbill for reimbursement?

    7. When and how will I be reimbursed?

What to do once you know your out-of-network benefits?

    • Appointments can be booked online, by phone, or in person

    • To book online, follow the link in our “Appointments'“ page

    • To book by phone or in person, we will simply need: Your first and last name, your phone number, email address, and a credit card to hold your appointment

    • After your physical therapy appointment, you will receive two emailed receipts:

      • One “simple” receipt stating the amount you paid, and the payment method (cash, check, credit/ debit card)

      • Another “detailed” receipt or “superbill.” This will contain information to submit to your insurance company for potential reimbursement. Items included will be a specific diagnosis code for your case, CPT codes, Tax ID, and your therapist’s license and NPI numbers.

  • If your insurance company has stated that you have out of network benefits:

    1. Submit your superbill to your insurance company as per their policy or instruction

    2. Await reimbursement in the form and timeline that they have provided you.
